Clinics and Services

New Patients
You will be asked to fill in a questionnaire before an appointment can be made for you to consult with our Practice Nurse to attend for a simple health check for which you will need to bring a small sample of urine.


pregnantMaternity Services
We offer shared care (your Doctor and the Hospital) during your pregnancy. Most expectant mothers have their antenatal checks at the surgery with a midwife. Antenatal clinics are held every Tuesday from 13.30 with the Midwife in attendance. She is available for any patients who may have any queries about their maternity care. She can also be contacted directly at Chorley Hospital Tel: 012572 45193 Post-natal checks are booked with the Doctors. Please contact the receptionists to make your appointment 6 weeks after the birth of your baby.


Family Planning Services
The Doctors and Practice Nurse are available for confidential advice on contraception matters. Please contact the receptionists for your appointment. Routine checks/repeat prescriptions for contraception are carried out by the Health Care Professionals. The practice does not provide routine coil fittings or contraception implants.


Child Health Surveillance
Baby Clinic Wednesday 10.00 – 12.00 Development checks are offered to all children at 6-8 weeks and at seven months, including immunisation. You will be seen by the Health Visitor, the Doctor and the Nurse if your baby is due for his immunisations. The Health Visitor can also be contacted on 01772 644100 for any advice you require for the well-being of your child. She is responsible for children from birth to age 5. Parents/Guardians will receive notifications via the local Community Health Services when the immunisations are due. Appointments are to be made with the Practice Nurse The clinic is held every Wednesday from 10.00 . Drop-in baby check-weigh is also available on this day.


Well-Person Check Up
For general screening, family planning advice, cervical smear and menopausal problems, smoking cessation. These appointments are available with the Practice Nurse by appointment.


Minor Surgery Clinic
Minor surgery is offered at the surgery, arrangements for which are initially requested by your Doctor. Please contact the receptionists for the range of minor surgery undertaken. The clinic is run by the Doctor and the Practice Nurse. The procedure will be explained to you and you will be asked to sign a consent form prior to any minor surgery.


Respiratory Clinic
It is recommended that patients with asthma and COPD are seen in the asthma clinic annually. This allows the nurse with specialist training to assess you, in order to help you to develop a self-management plan according to your needs. The clinic is run by the Practice Nurse.


diabetesDiabetic Clinic
It is recommended that patients with Diabetes attend for an annual appointment with the nurse for a thorough health check. You may already attend the hospital but the Doctors would like you to attend the surgery for an annual assessment. The clinic is run by the Practice Nurse.


Other Appointments

All patients who suffer from the following conditions, will be invited to attend the surgery on an annual basis, to see the Practice Nurse or Doctor to assess your condition and any change of treatment that may be required, or any assistance and information you may need.

  • Stroke
  • Hypertension
  • Epilepsy
  • Mental Health
  • Hypothyroidism
  • Cancer

Should you not want this assessment, please notify us, in writing. You will be removed from further invitations for the current year but will be invited to attend again the following year.


Counselling
A counselling service is available. You will be referred by the Doctors. There is a waiting list for this service. The counsellor will contact you directly with an appointment date and time.

Phlebotomist
A Phlebotomist attends the surgery every Friday from 8.30 to 11.30. Please make your appointment with the receptionists.
